The Case Manager will ensure
client charts and client software management system records are kept current and in compliance with all regulations. Advocate for clients to achieve goals and objectives within programs requirements. Act as liaison with Program Manager, Director, Funding Agencies, Families and Clients as necessary. Work with appropriate staff on methods used to meet client and program goals and objectives and ensure each client’s programs are being met.
Requisitos
This position requires a Clean DMV Record, valid CA Driver's License/with current insurance and a clean background check.
Bachelor’s Degree or commensurate experience (minimum four years) with developmentally disabled/mental health population or equivalent
Proficient with data entry skills with Word, Outlook and Excel programs
Bi-lingual skills required
