About the Role
Under the direction of the Individual Placement & Employment Services Program Manager, Case Manager will ensure client charts and client software management system records are kept current and in compliance with all regulations. Advocate for clients to achieve goals and objectives within program requirements. Act as liaison with program managers, directors, funding agencies, families, and clients as necessary. Work with appropriate staff on methods used to meet client and program goals and objectives and ensure each client’s program goals are being met.
Requirements
This position requires a Clean DMV Record, valid CA Driver's License, and a clean background check.
High School Graduate Required
Bachelor’s Degree preferred or commensurate experience (minimum of four years) with developmentally disabled/mental health population or equivalent.
Proficient with data entry skills, Outlook, and Microsoft Office products.
Excellent written, verbal, organizational skills,
Bilingual skills preferred
